Why Firms Bring Medical Chronologies In-House

Keep Records Secure

Client medical records stay in Lawft. Nothing is exported to a chronology vendor, and no additional party holds your clients’ protected health information.

Eliminate Turnaround

Processing begins the moment a record is marked medical. No uploading to another platform, no waiting on a reviewer, no re-filing the result.

Stay Current Automatically

When new records arrive, they are added to the existing chronology. No resubmission and no additional vendor fee.

How It Works

  • Mark records as medical. Files already in the case become numbered exhibits. PDFs, Word files, scans, faxes and photos are supported.
  • Medical AI reads every page. Each record is split into individual visits, and diagnoses, treatment, providers and dates are extracted.
  • Review a cited chronology. Every visit is tagged pre-incident or post-incident and links to the exhibit and page it came from.
  • Act on the flags. Pre-existing conditions, treatment gaps, non-compliance and other risks are surfaced with the quoted source text.

Medical AI Built for Clinical Records

General-purpose AI is trained on web text. Medical records are written in clinical shorthand, and misreading them produces errors that opposing counsel will find.

Recognizes negation. “Patient denies loss of consciousness” is recorded as no loss of consciousness, not as an injury.

Separates family history. A parent’s diabetes is not recorded as your client’s pre-existing condition.

Distinguishes suspected from confirmed. “R/O disc herniation” is not recorded as a diagnosis, and “consider MRI” is not recorded as a completed procedure.

Separates past from present. Prior medical history is identified, which drives the pre-incident and post-incident split.

Codes to medical standards. Diagnoses are linked to ICD-10-CM codes and medications to RxNorm.

Withholds uncertain findings. Low-confidence results are held for review rather than presented as fact.

What Every Chronology Includes

  • Visit-by-visit summaries with exhibit and page citations
  • Pre-incident and post-incident diagnosis tables with ICD-10 codes, severity and side-specific body parts
  • Emergency department timeline showing arrival, first treatment and time-to-treatment
  • Provider summary with specialty, visit dates, total visits and treatment status
  • Risk flags for pre-existing conditions, treatment gaps, substance use, non-compliance and record inconsistencies
  • Exports of the chronology as a PDF and the source exhibits as a ZIP file

Secure by Design

  • One system of record. No copies of client medical records at outside vendors.
  • Firm and case isolation. Records and extracted data are scoped to the firm and the case.
  • Encryption. Data is encrypted at rest and in transit, and the medical engine runs on infrastructure designed for protected health information.
  • Audit trail. Every record added to or removed from a chronology is logged.
  • Originals preserved. Source files are never altered.

Connected to the Rest of the Case

Document Management. The records already stored in the case are the exhibits. There is nothing to re-upload.

Settlements. Providers and medical bills carry into the PI settlement workbook and the settlement statement your client sees in the client portal.

Reporting. Medical expense, medical provider and settlement reports sit alongside the rest of your firm’s data.

Frequently Asked Questions

Why bring medical chronologies in-house instead of using a vendor?

Outsourcing requires exporting records, waiting on turnaround and resubmitting whenever new records arrive. Lawft processes records where they are already stored, keeps the chronology current automatically and keeps client medical information inside your firm’s system. Your team still reviews and finalizes every chronology.

How is Lawft’s AI different from general-purpose AI?

General-purpose AI is not designed for clinical documentation and can misread negated findings, family history or rule-out diagnoses as injuries. Lawft uses a medical language engine built for clinical text, and every finding is cited to its source page.

Can every entry be verified?

Yes. Every visit and every flag cites the exhibit and page it came from, and one click opens the source. The AI drafts the chronology; your team reviews it.

What happens when new records arrive?

Mark them as medical and they are added to the existing chronology. Only the new material is processed, and existing entries are not duplicated.

Which plan includes medical chronologies?

Medical chronologies are included in the Nimbus plan. See pricing for details.
